Please use this form to submit donations from individuals or corporate sponsors with payment by credit card.

If you would like to pay by check, you will need to use this form.

If you would prefer to submit the form by mail, please download and complete the Sponsorship/Donation Form PDF and send it, along with your check payable to POMS PTO, to the address on the form.
