How Do I Make My Commitment a Contribution?

• Option 1: One-time donation from one of the preset levels
• Option 2: One-time donation of a custom amount
• Option 3: Recurring monthly donation (for 8 total months) from one of the preset levels
• Option 4: Recurring monthly donation for a custom amount and duration
  
  

For Monthly Gifts Only:
By providing my credit card information, I authorize Pin Oak Middle School PTO, Inc. to charge my credit card on a recurring basis in the amount and terms as indicated below. I understand that this authorization shall remain in effect until I notify Pin Oak Middle School PTO, Inc. at least 30 days prior to the upcoming charge that I wish to end this agreement. I understand my credit card issuer may impose additional requirements and I should contact the credit card issuer for further information. A record of my contributions will appear on my billing statement.
