Socioeconomic Information Form

Have you submitted the socioeconomic information form? All our forms are important, but this one impacts our funding! Please make sure that this form is updated or completed for the 2021-2022 school year. It is very important that we receive this form from everyone.

Complete and print the Socioeconomic Information Form in English, Spanish, Vietnamese, or Arabic. Please return the signed form to the POMS front office or email to Mrs. Felissa Salinas at felissa.montanomarquez@houstonisd.org by September 20.
